Duplicate songs in Audio Media

I have tried clearing my media list and re-scanning. This only results in the duplicate songs again. I have tried renaming my Music folder and re-scanning with same results. If i manually delete each duplicate song on an album, it is fine until I do a re-scan, which brings back the duplicates.
Has anyone encountered this? Any solutions?

I might be thinking of something else, but I know I've seen behavior in some place where the music folders are scanned and each .mp3 file is detected, and then the .m3u files are parsed and that causes duplication of each file in the database.
